About The Position

The Registered Nurse (R.N.) Case Manager with General Inpatient (GIP) is responsible for the coordination and management of all patient care and other services for patients and their families in the community and in the hospital. The R.N. is responsible for the quality of care provided by the Hospice Home Health Aides. The R.N. supports and fosters the philosophy and mission of the organization, focusing on the physical, emotional, spiritual and medical/nursing needs of the patient and their family. The R.N. understands the culture of hospice, offering compassionate care to the patient at various stages of their illness. This position will work collaboratively with all interdisciplinary team members, including those in the hospital, to ensure patient and family needs are met. The R.N. will be clinically competent while delivering care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. R.N.s will effectively interact with patients, families and other health team members while maintaining all standards of professional nursing.

Requirements

  • Graduation from an accredited school of nursing.
  • Registered Professional Nurse with active current license in the State of Connecticut
  • Two years of recent experience as a staff nurse in an acute care hospital setting or hospice home care
  • Transportation required.

Nice To Haves

  • Bachelor of Science in nursing preferred, however registered nurses not holding BSN may be considered based on years of acute inpatient experience, including the care and management of hospice and palliative care patients.
  • HPNA/CHPN certification preferred.
