The RN Case Manager is responsible for initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs. This includes assessing for risks such as grief, and considering cultural and spiritual needs, as well as verbal and non-verbal communication. The role involves implementing and recommending revisions to the individualized plan of care, collaborating with patients, caregivers, physicians, and healthcare professionals. The RN Case Manager will attend Inter-Disciplinary Group (IDG) meetings, participate in patient care planning, and prioritize needs to promote coordinated care. Additionally, the position requires performing on-call responsibilities, timely submission of all documentation, and ensuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures. Other duties as required will also be performed.
