About The Position

The RN Discharge Planner is responsible for performing patient assessments, delivering Medicare messages in accordance with CMS guidelines, and utilizing community resources for adequate patient follow-up care. This role involves developing relationships with outside agencies, making appropriate referrals, and collaborating with the Utilization Nurse on tasks such as reviewing patients for medical necessity and coordinating patient care. The position also includes facilitating family meetings, documenting discharge planning and social services, and demonstrating flexibility, organization, and the ability to function under stressful situations. The RN Discharge Planner will consult with other departments for patient care and performance improvement, treat patients and families with respect, and communicate patient needs and plans to family members and caretakers. The role requires the ability to work independently and within a team, relate well with various stakeholders, and accurately determine and provide necessary assistance and resources for patients and families. Maintaining good inter- and intra-departmental relationships and ensuring documentation meets current standards are also key aspects of this role.

Requirements

  • Current Registered Nurse licensure in the State of Utah
  • Motivated, independent individual who can organize workload so that all functions are completed appropriately
  • Working knowledge of the criteria for Medicare, Medicaid, HMO, and private insurance coverage
  • Experience in providing patient care

Responsibilities

  • Performing needed assessments on patients
  • Delivering Medicare messages by staying up to date with CMS guidelines
  • Utilizing community resources to ensure that the patients have adequate follow-up care
  • Developing relationships with outside agencies and making referrals as appropriate
  • Reviewing patients for medical necessity and coordination of patient care (shared with Utilization Nurse)
  • Coordinating and facilitating family meetings to discuss care
  • Documenting discharge planning and social services in an ongoing manner
  • Demonstrating the ability to be flexible, organized, and function under stressful situations
  • Consulting other departments as appropriate to collaborate on patient care and performance improvement activities
  • Treating patients and families with respect and dignity
  • Communicating with family members and caretakers regarding the needs of the patient and anticipated plans
  • Working well independently and within a multi-faceted team environment
  • Relating well with multiple stakeholders, including physicians, nurses, allied health professionals, outside agencies, and patients
  • Accurately determining the type of assistance/setting/resources necessary for the patient/family and providing appropriate resources/assistance list of facilities
  • Maintaining a good working relationship both within the department and with other departments
  • Ensuring documentation meets current standards and policies
  • Ensuring that appearance and personal conduct are professional at all times
  • Maintaining an excellent attendance record
