The RN Discharge Planner is responsible for performing patient assessments, delivering Medicare messages, and utilizing community resources to ensure adequate follow-up care. This role involves developing relationships with outside agencies, making referrals, and collaborating with the Utilization Nurse on patient care coordination and medical necessity reviews. The position requires facilitating family meetings, documenting discharge planning and social services, and demonstrating flexibility, organization, and the ability to function under stress. The RN Discharge Planner will consult with other departments for patient care and performance improvement, treat patients and families with respect, and communicate care plans to family members and caretakers. The role requires working independently and within a team, relating well with various stakeholders, and accurately determining patient needs for discharge. Maintaining good working relationships and ensuring documentation meets standards are also key responsibilities.
