About The Position
Bayshore Medical Personnel is seeking a full-time contract RN Case Manager (Care Coordinator) in Winnipeg, MB. This role involves providing services to members with protracted and complex health issues. RN Case Managers work within a multidisciplinary primary health care team in a primary care/clinic setting. Their main duties include conducting detailed case management assessments, creating comprehensive care plans tailored to individual health needs, and monitoring clients to suggest necessary modifications.
Requirements
- At least 3 years of clinical experience within the last 4 years in nursing practice.
- Previous case management experience.
- Baccalaureate Degree in Nursing from a recognized university.
- Certificate of Basic Life Support (BLS) for Healthcare Provider or CPR/AED Level C. Annual recertification is required.
- Current license to practice from a provincial or territorial College of Nurses in the province of practice.
Responsibilities
- Complete detailed case management assessments.
- Develop comprehensive care plans that reflect individual health care needs.
- Monitor clients and make suggestions for modifications as necessary.
