About The Position

The RN Case Manager & Care Coordinator, Team Lead manages all PACE participants in the ambulatory setting. Actual salary offers may vary based on factors including budget, experience, skills, education, licensure, certifications, and other business considerations; the range may change. The role may also qualify for discretionary bonuses or incentives.

Requirements

• Current valid Registered Nurse license through the California Board of Registered Nursing.
• Valid Basic Life Support (BLS) certification or higher, following American Heart Association (AHA) or American Red Cross guidelines.

Nice To Haves

• At least 1 year of geriatric nursing experience is preferred.
• At least 1 year of experience in a managed care setting is preferred, preferably in utilization or case management in an ambulatory or inpatient setting.
