Requirements

Active Illinois RN license
Strong background in care management as well as complex and chronic condition management
Skilled in motivational interviewing, patient education, and patient advocacy
Ability to work independently in a remote environment while staying connected to the team
Comfort with technology and multiple EMR systems
Minimum of five (5) years of clinical experience across diverse healthcare settings
Behavioral Health: Requires advanced knowledge in psychiatric conditions, crisis intervention, and therapeutic communication
