HOPE Community Services is seeking a Registered Nurse (RN) Nurse Care Manager to support their Urgent Recovery Center (URC), Intensive, Community Living, and Children & Youth programs. This position is ideal for an RN who has strong clinical judgment, enjoys working in a fast-paced environment, and is passionate about providing coordinated, client-centered care. It is a dual position, providing on-site nursing support within the URC while also supporting care coordination across programs. The RN Nurse Care Manager plays a key role in ensuring safe, high-quality, and compliant nursing care, while supporting integrated, recovery-focused services for the individuals we serve. This position serves as an on-site nursing resource within the URC while maintaining meaningful involvement in care coordination. This dual RN role combines direct clinical support within the Urgent Recovery Center (URC) with nurse care management responsibilities across programs. During scheduled shifts, the RN serves as an on-site clinical resource, providing assessment, guidance, and escalation support to promote safe, high-quality care aligned with best practices and regulatory standards. As part of the URC’s integrated care team, the RN works in a 24/7 environment supporting individuals experiencing acute behavioral health crises. This role supports both medical and behavioral health interventions and plays a critical part in ensuring care is safe, timely, and trauma-informed. In addition to clinical responsibilities within the URC, the RN supports care management activities for Intensive, Community Living, and Children & Youth programs. This includes completing health assessments, coordinating care, and collaborating with interdisciplinary team members, Hope outpatient services, the Medication Clinic, and community healthcare providers.
