Requirements

Active and unrestricted RN license in the state of Virginia
Minimum of 1-2 years of home health or clinical nursing experience preferred
Proficiency with OASIS documentation and Medicare home health regulations
Strong clinical assessment and critical thinking skills
Excellent communication, time management, and organizational abilities
Reliable transportation and valid driver's license for home visit travel
Comfortable working independently in a field-based, patient-facing environment

Responsibilities

Conduct comprehensive in-home patient assessments to determine care needs and eligibility for home health services
Complete OASIS assessments, start-of-care evaluations, and recertification assessments in compliance with regulatory standards
Collaborate with physicians, care teams, and family members to develop and communicate individualized care plans
Document clinical findings accurately and in a timely manner using electronic health records (EHR)
Ensure compliance with Medicare, Medicaid, and Joint Commission guidelines during all assessment activities
Communicate patient status changes and clinical concerns to the appropriate care team members
Maintain a flexible schedule to accommodate patient and agency assessment needs across the Arlington, VA service area
