About The Position

Navitas Healthcare, LLC is seeking a Registered Nurse – Case Manager for an exciting Travel or Local job in Eureka, CA. This role involves performing case management duties for acute care patients, including assessment, planning, and coordination of care. The Case Manager will develop individualized care plans in collaboration with physicians, nurses, and interdisciplinary teams, and facilitate patient transitions, including discharge planning, referrals, and follow-up care coordination. Communication with patients, families, and care providers regarding treatment plans and care needs is essential. The role also requires ensuring compliance with facility policies, procedures, and regulatory standards, and participating in unit assignments and floating between units as needed to support patient care. Accurate and timely documentation in the Epic electronic medical record system is expected, as is assisting patients with resources and services, including housing support for travelers if required.

Requirements

  • Active Registered Nurse (RN) license in California.
  • Minimum of 2 years of acute care nursing experience (first-time case managers may be considered with clinical approval).
  • Strong communication, organizational, and critical thinking skills.
  • Ability to work every other weekend as scheduled.
  • Proficiency with electronic medical records, preferably Epic.
  • Ability to float between units based on staffing needs.

  • Experience in case management or care coordination preferred.
