RN Care Transition Coordinator
About The Position
The RN Care Transition Coordinator provides care transition services for Providence hospitalized patients. The Coordinator works with the Providence physician team and hospital discharge planning staff to help ensure patients are successfully discharged from the hospital without disruption in services. The role includes nursing assessment, liaison work, communication, education, and team and family support. The position is remote, but the employee is required to attend in-person training and required education and to access equipment. The role is with Physician Management Group and supports Providence’s patient-focused, whole-person care mission.
Requirements
- Graduate from an accredited school of Nursing.
- Washington Registered Nurse License upon hire.
Nice To Haves
- Bachelor's Degree in nursing.
- 1 year of clinical experience.
- Experience providing patient and staff education.
- Experience in conflict resolution.
Responsibilities
- Provide care transition services for Providence hospitalized patients.
