RN Care Manager - PACE
About The Position
The Registered Nurse Care Manager establishes a well-rounded care practice that includes patient assessment, care planning, implementation, coordination, monitoring, and ongoing routine evaluation to ensure medically and financially appropriate care. Responsibilities may also include off-site or community visits to participants to provide care, wound care, foot care, and lab draws. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to help older adults remain as healthy as possible while living in the community through clinics, home visits, and other services. The team helps empower elders in the community to live active, independent lives. The schedule is Monday through Friday on the day shift. A $5,000 hiring bonus is available for eligible external candidates who meet required qualifications and payment conditions.
Requirements
- Education sufficient for clinical licensure.
- Washington Registered Nurse License upon hire.
- National Provider BLS certification from the American Heart Association upon hire.
- One year of long-term care or geriatric nursing experience.
Nice To Haves
- Bachelor’s Degree in Nursing.
- Medical-surgical or rehabilitation medicine experience.
Responsibilities
