The RN Care Manager will conduct thorough assessments of patients' medical, social, and psychological needs. They will collaborate with healthcare providers to develop comprehensive care plans tailored to individual patient goals, identify and address barriers to care, and coordinate services accordingly. This role serves as a liaison between patients, their families, healthcare providers, and community resources to facilitate the seamless delivery of care. The RN Care Manager will monitor and manage patient progress through the healthcare continuum, coordinate referrals to specialists, arrange for home healthcare services, and ensure timely appointments and treatments. They will oversee transitions of care, including admissions and discharges, and provide patients and families with information about their diagnoses, treatment options, and available support resources. Additionally, the RN Care Manager will educate patients on self-management strategies and empower them to actively participate in their care decisions. This role requires maintaining accurate and up-to-date electronic health records, documenting all interactions, assessments, and interventions, and preparing and presenting reports to communicate patient progress and outcomes to the healthcare team. Collaboration with the interdisciplinary team to identify opportunities for improving care processes and patient outcomes, participation in quality improvement initiatives, and ensuring compliance with all relevant healthcare regulations and standards, including HIPAA, are also key responsibilities.
