About The Position

The RN Care Manager - Providence Child Center, Long Term Care in Portland, Oregon, works within a 46-bed unit serving both pediatric and adult patients. This unit cares for medically fragile individuals, including children with complex developmental and neurological conditions and adults with chronic illnesses and disabilities. Some residents require long-term ventilator support and tracheostomy care, and all need assistance with daily living activities. The team is committed to compassionate, resident-centered care in a nurturing environment, emphasizing teamwork, respect, and continuity of care. The role involves an interdisciplinary approach, collaborating with respiratory therapy, rehab, and social work. Providence offers a comprehensive benefits package. The Resident Care Manager is accountable for the quality of care for children, following the nursing process and Nurse Practice Act, managing nursing care to ensure maximum resident independence and self-direction. Requirements

  • Bachelor's Degree in Nursing.
  • Upon hire: Oregon Registered Nurse License.
  • Upon hire: In compliance with the Department of Human Services (DHS), a criminal background check is required for this position (every 2 years).
  • 3 years of Nursing experience, with 2 years in care of medically fragile children and adults
  • Recent relevant experience.

Nice To Haves

  • Master's Degree in Nursing.
  • 1 or more years of Care/case management experience.
  • Leadership experience.

Responsibilities

  • Accountable for quality of care for children, according to the individualized needs of the child, following the nursing process and in accordance with the Nurse Practice Act.
  • Manages the nursing care of assigned residents, ensuring their maximum independence and self-direction.
  • Coordinates care provision with physicians and other Interdisciplinary Team members in a collegial relationship.
  • Ensures that the resident's care plan is developed, implemented and documented to meet their individual care needs.
  • Accepts professional development as an integral part of nursing practice.
