About The Position

Regions Hospital is seeking an experienced RN to join their Care Management department in an RN Care Manager position. This role provides inpatient vs Hospital care management services for medical/surgical patients, aiming to facilitate optimal patient outcomes, support effective and efficient care delivery, improve continuity of care, and ensure efficient use of resources to progress the plan of care. A primary focus is on developing a patient-focused discharge plan to prevent readmissions. The RN Case Manager will collaborate with a multidisciplinary care team, including attending/specialty physician(s), Social Work, Utilization Review, therapies, and Nursing, to ensure timely and effective treatment plans are implemented and appropriate discharge plans lead to smooth transitions in levels of care. The position requires working day shifts, every 4th weekend, and one holiday per year. Requirements

  • Associate’s degree of nursing with RN licensure from an accredited college
  • One (1) year working as a registered nurse
  • Current State of Minnesota Nursing Board RN license

Nice To Haves

  • Bachelors or Master’s degree of Nursing
  • Three years’ experience
  • Experience performing case management in Home Care, hospital, Post-Acute Care, discharge planning, care and/or quality improvement
  • Certified Case Manager through Commission for Case Manager Certification (CCMC) or eligibility to sit for CCM exam within 2 years of hire

Responsibilities

  • Provide inpatient vs Hospital care management services for medical/surgical patients to facilitate optimal patient outcomes
  • Support the delivery of effective and efficient care
  • Improve continuity of care and efficient use of resources to progress the plan of care
  • Focus on developing a discharge plan to prevent readmissions and ensure it is patient-focused
  • Collaborate with the care team including, but not limited to: attending/specialty physician(s), Social Work, Utilization Review, therapies, and Nursing to ensure that an effective and timely treatment plan is acted upon
  • Ensure appropriate discharge plan is implemented with a smooth transition in levels of care
  • Provide Care Management services on weekends and/or holiday(s) rotations as determined by leadership
