This role supports the Healthy Behavior and Function Clinic - part of the Hub and Spoke program in Spokane. You will support patients with opiate, alcohol and methamphetamine use in a care management capacity providing resources and care coordination. The Nurse Care Manager provides skilled nursing services to patients with opioid use disorders and facilitates coordination of an interdisciplinary plan of care to guide patient progress toward achieving clinical, quality, and cost-effective outcomes. The Nurse Care Manager is an integral member of the Hub and Spoke multidisciplinary network supporting evidence based opioid use disorder treatment. The Nurse Care Manager provides assessments, evaluation, triage, teaching, medication induction, stabilization, and monitoring of patients in the grant funded opioid treatment (OTP) program. He/she collaborates with OTP team members and outside referral sites, works with medical director and DATA-waived providers when treating patients, and with other addiction and mental health programs and primary care providers in assisting patients in recovery and supporting their referral needs. The Nurse Care Manager operates independently, working cooperatively and collaboratively with patient, families, health care providers, payers, managers, and network partners. The Nurse Care Manager provides nursing diagnosis, counseling, education, and follow-up to patients served by these programs and makes assessments of potentially eligible persons and follow-up care as indicated by their assessment.
