RN, Care Coordinator - Care Connections
About The Position
Saint Luke's Hospital is looking to add a RN, Care Coordinator to the Care Connections team. This is a full-time opportunity in an in-patient environment. The ideal candidate will have previous care coordination or case management experience in an acute care setting. BSN Degree required within 3 years of start of position.
Requirements
- 1 year of previous care coordination or case management experience in an acute care setting.
- Basic Life Support - American Heart Association or Red Cross.
- Registered Nurse.
- Bachelor's Degree - Science in Nursing.
- BSN Degree required within 3 years of start of position.
Responsibilities
- Work with the patient to focus on collaboration between hospital and community-based providers and arranging post discharge services.
- Function as a member of a multidisciplinary team with a goal of improving the patient’s health status and reducing the need for hospital care.
- Assistance with program development, evaluation, professional education, or supervision of students.
