RN, Care Coordinator - Care Connections
About The Position
Saint Luke's Hospital is looking to add a RN, Care Coordinator to the Care Connections team. This is a full-time opportunity for a dynamic, motivated individual driven to create healthier communities through excellent patient care. The ideal candidate will have previous care coordination or case management experience in an acute care setting. A BSN Degree is required within 3 years of the start of the position. The Care Connections Nurse is part of a team designed to effectively assist patients in transitioning from hospital to community. The nurse will work with the patient to focus on collaboration between hospital and community-based providers and arranging post discharge services. The Care Connections Nurse functions as a member of a multidisciplinary team with a goal of improving the patient’s health status and reducing the need for hospital care. Duties may also include assistance with program development, evaluation, professional education, or supervision of students.
Requirements
- 1 year of applicable experience
- Basic Life Support - American Heart Association or Red Cross
- Registered Nurse - Various
- Bachelor's Degree - Science in Nursing
- BSN Degree required within 3 years of start of position.
