M Physicians' Orthopedic Surgery specialists provide care for a broad spectrum of bone and joint injuries, treating conditions affecting ankles, feet, hands, shoulders, elbows, and spine, as well as musculoskeletal tumors. The RN Care Coordinator will coordinate care across the health care continuum within designated sub-specialty population(s). This role serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services. The coordinator will also complete pre-visit planning and post-visit wrap-up, determine vital follow-up appointments and/or treatments to optimize patient visits, and facilitate proactive disease management and preventative health care using evidence-based practice. Additionally, the role involves rooming patients for the surgeon(s) as needed, completing clinic readiness including records review, and providing administrative support.
