About The Position
Saint Luke’s is seeking a dynamic, motivated registered nurse for a full-time position in their in-patient environment. Saint Luke’s is a faith-based, not-for-profit health system dedicated to providing excellent compassionate health care and health-related services, focusing on enhancing the physical, mental, and spiritual health of the communities they serve. This role requires a BSN degree within 3 years of the start of the position. The Care Connections Nurse is part of a team focused on assisting patients with transitions from hospital to community, collaborating with providers, and arranging post-discharge services. This role functions within a multidisciplinary team to improve patient health status and reduce hospital readmissions. Duties may also include program development, evaluation, professional education, or student supervision.
Requirements
- 1 year of applicable experience
- Basic Life Support - American Heart Association or Red Cross
- Registered Nurse - Various
- Bachelor's Degree - Science in Nursing

Responsibilities
