RN/Behavioral Health Home Program Supervisor
About The Position
Behavioral Crossroads, LLC is seeking a part-time RN/Behavioral Health Home Program Supervisor to join their team. This is an in-office position with a pay range of $35-$40 per hour. The role involves managing a Behavioral Health Home team, coordinating care, participating in meetings, conducting assessments, screening clients, developing wellness programming, engaging with participants, collaborating with community providers, and overseeing care planning. The supervisor will also assess wellness needs, participate in treatment planning, assist with data collection, develop WRAP services, manage transportation to appointments, work with billing and finance, ensure regulatory compliance, provide coverage for mental health groups, coordinate on-call support, and carry a client caseload.
Requirements
- Must be licensed in NJ as an RN or RPN
- Must possess a Bachelor’s degree in a human services field.

- Three years of related experience in a Behavioral Health setting.
Responsibilities
- Manage the Behavioral Health Home team with care coordination and care transitioning.
- Participate in daily rounding meetings.
- Works with APN to complete health assessments on clients of Behavioral Health Home.
- Manage the screening of potential clients for eligibility for Behavioral Health Home.
- Participate in the development of wellness programming for program participants.
- Conduct outreaches and engagement activities with program participants.
- Collaborates with providers in the community who are involved with clients in the behavioral health home program.
- Coordinates the clients care with community treatment providers, and oversees care planning.
- Assess program participant’s wellness needs.
- Conduct wellness and engagement activities for program participants.
- Participate in treatment planning with the behavioral health team.
- Assist with required data collection on clients enrolled in the behavioral health home.
- Develop and implement WRAP services to program participants.
- Manage with transporting consumers to needed appointments and services.
- Work closely with billing and finance to ensure coordinated billing process.
- Meets or exceeds contracted commitments including budgeted volume of service.
- Ensure full compliance with regulatory requirements, policies, and procedures.
- Provide coverage and support for Behavioral Crossroads Mental Health groups.
- Coordinate 24/7 on call support for BHH consumers.
- Carries a BHH and partial care client caseload.
