About The Position

We are seeking an experienced Registered Nurse (RN) to provide high-quality nursing care in a busy ambulatory and primary care setting. The RN will support outpatient healthcare services by performing patient assessments, coordinating care plans, administering medications and treatments, assisting providers with clinical procedures, and promoting positive patient outcomes. The ideal candidate will possess strong primary care experience, excellent clinical assessment skills, and the ability to work independently while delivering compassionate, patient-centered care.

Requirements

  • Associate Degree in Nursing (ASN/ADN) from an accredited nursing program preferred.
  • Active New York State Registered Nurse (RN) License required.
  • Current BLS Certification required and must be maintained throughout assignment.
  • Minimum one (1) year of recent RN experience required.
  • Recent experience within the last two years in an acute care facility or similar healthcare setting required.
  • Primary Care and Ambulatory Nursing experience required.
  • Strong nursing assessment skills required.

Nice To Haves

  • Ambulatory Care experience.
  • Urgent Care experience.
  • Primary Care or Family Medicine background.
  • Experience using EPIC Electronic Medical Records.
  • Experience working within a Level I Trauma healthcare system.

Responsibilities

  • Provide professional nursing care in an ambulatory and primary care environment.
  • Conduct comprehensive patient assessments and identify healthcare needs.
  • Monitor patient conditions and communicate findings to providers.
  • Develop and implement appropriate nursing interventions and care plans.
  • Support preventive care initiatives and patient wellness programs.
  • Assist physicians and advanced practice providers with examinations and clinical procedures.
  • Administer medications, injections, and prescribed treatments.
  • Perform nursing procedures including IV therapy and patient education.
  • Monitor patient responses to treatments and interventions.
  • Coordinate patient care plans and follow-up services.
  • Facilitate referrals and communication between healthcare providers and specialists.
  • Collaborate with multidisciplinary healthcare teams to ensure continuity of care.
  • Support efficient patient flow and clinic operations.
  • Maintain accurate and timely documentation within electronic medical record systems.
  • Ensure compliance with healthcare regulations, facility policies, and infection control standards.
  • Protect patient confidentiality and maintain HIPAA compliance.
  • Participate in quality improvement and patient safety initiatives.
  • Educate patients and families regarding medications, treatments, preventive care, and disease management.
  • Provide guidance on follow-up care and healthy lifestyle practices.
  • Promote patient engagement and self-management strategies.
