About The Position

We are seeking an experienced Registered Nurse (RN) – Adult Care / Home Care Nurse to provide high-quality nursing services to adult patients within the New York City home care setting. The RN will deliver comprehensive patient-centered care, conduct clinical assessments, administer treatments, and collaborate with interdisciplinary healthcare teams to ensure continuity of care and positive patient outcomes.

Requirements

Associate Degree in Nursing (ADN/ASN) or higher.
Active New York State RN License.
Current AHA BLS Certification.
Minimum 2 years of recent Medical-Surgical nursing experience.
Previous Home Care nursing experience.
OASIS documentation experience required.
Valid Driver's License.
COVID Vaccination Documentation.

Nice To Haves

EPIC EMR experience preferred.
Prior NYC Home Care experience.
Strong case management and patient education skills.
Experience managing complex adult patient populations.

Responsibilities

Assess, plan, implement, and evaluate individualized patient care plans.
Provide direct nursing care to adult home care patients.
Administer prescribed medications and treatments.
Perform wound care, IV therapy, catheter care, and post-operative nursing services.
Monitor patient conditions and report significant changes appropriately.
Educate patients and families regarding disease management and home care instructions.
Maintain accurate clinical documentation and patient records.
Collaborate with physicians, therapists, case managers, and interdisciplinary teams.
Participate in discharge planning and continuity of care initiatives.
Follow infection prevention standards and nursing best practices.
Complete OASIS documentation and utilize electronic medical records as required.
