About The Position
Navitas Healthcare, LLC is seeking an experienced RN – Adult Care / Home Care Nurse to provide high-quality nursing services to patients in their homes throughout the Bronx Borough. The RN will deliver comprehensive patient-centered care, conduct clinical assessments, administer treatments, and collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal patient outcomes.
Requirements
- Minimum 2 years of recent Med/Surg experience required
- Home care nursing experience required
- OASIS knowledge required
- EPIC EMR experience preferred
- Must be comfortable traveling to patient homes within Bronx Borough
- Active New York State RN License required
- Primary Source Verification of RN License required
- AHA BLS Certification required
Nice To Haves
- EPIC EMR experience preferred
Responsibilities
- Assess, plan, implement, and evaluate individualized patient care plans
- Provide direct nursing care to adult home care patients
- Administer prescribed medications and treatments
- Perform wound care, IV therapy, catheter insertion, and post-operative care
- Monitor patient vitals and report adverse reactions or changes in condition
- Educate patients and families regarding treatment plans and home care instructions
- Maintain accurate documentation, patient files, and health records
- Collaborate with healthcare providers on discharge planning and continuity of care
- Ensure adherence to nursing standards, infection control, and clinical protocols
- Utilize OASIS documentation and EPIC EMR systems as required
