About The Position

Identify, assess and admit appropriate patients for hospice services. RN Admissions - PRN. This position is PRN and has a flexible job shift. The RN Admissions role is part of the Access & Marketing department. The RN will perform the admissions process, which includes accepting patient referrals, visiting patients in various settings (residential, nursing home, hospital), performing assessments (vital signs, pain assessment, breath sounds), determining appropriateness for admissions, completing admission documentation, consulting with supervisors and MDs, and referring patients back to the referral source if needed. The role also involves teaching patients and caregivers about end-of-life care, initiating the Plan of Care, and providing psychosocial support. Additionally, the RN will coordinate patient care, including personal care, DME, and obtaining medications/supplies, document assessments and progress notes in a timely manner, and act as a liaison between patients, physicians, care team members, and referral sources. Other duties may be assigned.

Requirements

  • Graduation from an approved school of professional nursing as determined by the Board of Nurse Examiners required.
  • Licensed as a Registered Nurse by the Texas Board of Nurse Examiners required.
  • Valid Texas driver’s license, auto liability insurance and safe driving record required.
  • CPR certification required.
  • 1-year previous hospice experience.

Responsibilities

  • Performs admissions process.
  • Accepts patient referral; visits patient in residential, nursing home or hospital setting.
  • Performs assessments, i.e. vital signs, pain assessment, breath sounds, etc.
  • Determines appropriateness for admissions.
  • Completes documentation for admission to services if appropriate.
  • Consults with supervisors, MDs, etc. regarding appropriateness for hospice services.
  • Refers patient back to referral source for additional follow-up as needed.
  • Teaches patient and caregivers (including nursing home staff) on end of life care – e.g. disease process, death and dying, comfort measures, medication use, side effects, etc.
  • Initiates Plan of Care.
  • Provides psychosocial support to patients and families.
  • Initiates coordination of personal care, DME, and obtaining medications and medical supplies.
  • Documents in a timely manner patient assessments, patient contacts, physician orders and progress notes.
  • Acts as a liaison between patient and physicians, other care team members and referral sources.
