About The Position

Home Health Care, Inc. is seeking an experienced Home Health RN to focus specifically on weekend Start of Care admissions. This role offers a unique opportunity to utilize your expertise in admissions and OASIS documentation without the commitment of a traditional 12-hour shift or ongoing caseload management. You will be compensated $200 per Start of Care visit plus mileage reimbursement, allowing you to earn meaningful additional income while maintaining your weekdays free or continuing your current Monday through Friday position. This role is ideal for experienced home health nurses who excel at comprehensive assessments, clinical decision-making, and accurate OASIS documentation.

Requirements

Active Minnesota Registered Nurse license.
Current CPR certification.
Previous home health nursing experience.
Strong OASIS knowledge.
Start of Care or admission experience strongly preferred.
Strong assessment and clinical decision-making abilities.
Excellent documentation and communication skills.
Valid driver's license, active insurance, and reliable transportation.

Responsibilities

Complete weekend Start of Care admissions for new home health patients.
Perform comprehensive skilled nursing assessments.
Complete OASIS and admission documentation accurately and on time.
Review medications, diagnoses, symptoms, safety concerns, and caregiver needs.
Develop and initiate individualized plans of care.
Provide patient and caregiver education.
Identify additional clinical and therapy needs.
Coordinate care with physicians, nurses, therapists, and other interdisciplinary team members.
Communicate significant clinical findings and changes appropriately.
