Admissions RN (Registered Nurse) – In -Home Intake & Assessment

Requirements
- Active Minnesota RN license
- Strong time management and independent work style
- Comfortable working in a field-based, patient-centered role
- Reliable transportation
Nice To Haves
- Experience with home health admissions and OASIS preferred
Responsibilities
- Complete Start of Care (SOC) admissions in patients’ homes.
- Conduct detailed clinical assessments and OASIS documentation.
- Initiate care plans and ensure a smooth transition into services.
- Educate patients and families on what to expect moving forward.
- Coordinate with the care team to set patients up for success.
