About The Position

Home Health Care, Inc. is seeking an experienced home health RN to focus specifically on weekend Start of Care admissions. This role offers a unique opportunity to utilize your assessment and clinical decision-making skills for meaningful additional income without the commitment of a traditional 12-hour shift or ongoing caseload management. You will be compensated $200 per Start of Care (SOC) visit plus mileage reimbursement. This is an ideal opportunity for nurses who are already working full-time and wish to supplement their income with premium weekend earnings, or for part-time nurses looking to add more visits. Military medical experience is highly valued, and the company proudly welcomes Veterans, Reservists, National Guard Members, and Active Duty Military Personnel.

Requirements

• Active Minnesota Registered Nurse license.
• Current CPR certification.
• Previous home health nursing experience.
• Strong OASIS knowledge.
• Start of Care or admission experience strongly preferred.
• Strong assessment and clinical decision-making abilities.
• Excellent documentation and communication skills.
• Valid driver's license, active insurance, and reliable transportation.

Nice To Haves

• Military medical experience is highly valued.
• Applicable military codes include Army 66H, 66N, 66R; Navy 290X, 1960; Air Force 46AX, 46FX; Marines MED; and Coast Guard HS.

Responsibilities

• Complete weekend Start of Care admissions for new home health patients.
• Perform comprehensive skilled nursing assessments.
• Complete OASIS and admission documentation accurately and on time.
• Review medications, diagnoses, symptoms, safety concerns, and caregiver needs.
• Develop and initiate individualized plans of care.
• Provide patient and caregiver education.
• Identify additional clinical and therapy needs.
• Coordinate care with physicians, nurses, therapists, and other interdisciplinary team members.
• Communicate significant clinical findings and changes appropriately.
