About The Position

As an Admissions Nurse for our Hospice team, you will be responsible for traveling to CHLs and homes to see hospice clients and their families. Your duties will include completing various assessments such as initial, resumption, follow-up, significant change of status, and discharge assessments. You will also initiate and update individualized plans of care, determine nursing diagnoses based on health assessment data, and implement physician's orders. Additionally, you will initiate preventive and rehabilitative nursing procedures, administer medications, counsel patients and families, provide health care instructions, and assist patients with daily living activities to promote self-sufficiency and comfort.

Requirements

  • Registered Nurse (RN) license
  • Experience in hospice care
  • Ability to travel to patient homes and CHLs
  • Proficiency in conducting comprehensive head-to-toe assessments
  • Skill in developing and updating individualized plans of care
  • Knowledge of nursing diagnoses and health assessment data interpretation
  • Ability to obtain, implement, and maintain physician's orders
  • Competence in administering medications and treatments
  • Strong counseling and patient/family education skills
  • Ability to assist patients with activities of daily living

Responsibilities

  • Traveling to CHLs and homes to see hospice clients and their families
  • Complete initial, resumption, follow up, a significant change of status and discharge assessments including comprehensive head to toe assessment, HIS assessment, and all associated assessments with admission or discharge
  • Initiate an individualized plan of care, and update the plan of care for resumptions, significant change in status, and re-certifications.
  • Use health assessment data to determine nursing diagnosis
  • Obtain, implement, perform and maintain orders as received by physicians overseeing patient care services
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed by the physician
  • Counsel the patient and family in meeting nursing and related needs
  • Provide health care instructions to the patient as appropriate per assessment and plan of care
  • Assist the patient with activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
