About The Position

The Hospice Admissions Registered Nurse (RN) plays a critical role in providing compassionate and comprehensive care to patients and their families during the transition to hospice services. This position is responsible for conducting thorough patient assessments to determine eligibility and appropriateness for hospice care, ensuring that patients receive timely and personalized support. Requirements

• Current and valid Registered Nurse (RN) license in the state of practice.
• Minimum of 2 years clinical nursing experience, preferably in hospice, palliative care, or related fields.
• Strong knowledge of hospice care principles, patient assessment, and symptom management.
• Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare teams.
• Ability to work independently and manage multiple priorities in a fast-paced environment.
• Clinical assessment skills daily to evaluate patient conditions and determine hospice eligibility accurately.
• Strong communication skills are essential for educating patients and families about hospice care options and addressing their concerns with empathy and clarity.
• Organizational skills enable the RN to manage documentation, coordinate admissions, and collaborate efficiently with interdisciplinary teams.
• Critical thinking and problem-solving abilities support the development of individualized care plans that meet complex patient needs.

Nice To Haves
