About The Position

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team as a Start of Care (SOC) Nurse. The SOC Nurse is responsible for completing comprehensive patient assessments, developing individualized plans of care, coordinating services, and ensuring patients meet eligibility requirements for home health services. This role is ideal for an experienced nurse who enjoys independent practice, critical thinking, and patient-centered care in the home setting. This is a Part-Time, Pay Per Visit Position

Requirements

  • Current Registered Nurse (RN) license in the State of Minnesota.
  • Graduate of an accredited nursing program.
  • Minimum one year of nursing experience required; home health experience preferred.
  • OASIS experience preferred or willingness to complete training.
  • Strong assessment, communication, and organizational skills.
  • Ability to work independently and manage time effectively.
  • Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

  • Complete Start of Care (SOC), Resumption of Care (ROC), and Discharge assessments in patient homes.
  • Perform comprehensive physical, psychosocial, and environmental assessments.
  • Complete OASIS assessments accurately and within required timeframes.
  • Establish individualized plans of care in collaboration with physicians and interdisciplinary team members.
  • Identify skilled nursing needs and coordinate appropriate home health services.
  • Educate patients and caregivers regarding diagnoses, medications, disease management, safety, and treatment plans.
  • Communicate significant findings and obtain physician orders as needed.
  • Ensure documentation is accurate, timely, and compliant with Medicare, state, and agency requirements.
  • Collaborate with nursing, therapy, and home health aide staff to ensure continuity of care.
  • Participate in agency meetings, quality improvement activities, and ongoing education.
