About The Position

Community Healthcare of Texas provides hospice and palliative care services to individuals living with serious illnesses and supports their loved ones. The RN Admission Nurse meets with patients and families to explain hospice services and coordinate patient admissions. The position includes assessing patients at admission, coordinating initial nursing visits and care needs, communicating with the Interdisciplinary Team, and completing admission documentation. The schedule is Friday through Sunday, 8:00 a.m. to 8:30 p.m.

Requirements

• Registered Nurse in the state of Texas.
• Valid driver’s license.
• At least 1 year of nursing experience.
• Case Management experience.
• Proficient computer skills and experience with Microsoft Office Suite.

Nice To Haves

• Strongly preferred: 2 or more years of experience in hospice case management.

Responsibilities

• Conduct admission consultations and educate patients and families about hospice services.
• Complete a thorough patient assessment at admission; establish or arrange the initial frequency of nursing visits, durable medical equipment and prescription needs, and assess the need for bereavement, social work, chaplain, and volunteer services.
• Attend and report to the Interdisciplinary Team regarding the patient.
• Complete paperwork and charting thoroughly and accurately according to policy.
• May conduct nursing or “tuck-in” visits.
• May train other admission staff on admission processes.
• May conduct supervisory visits for LVN or CNA staff.
