About The Position

Community Healthcare of Texas provides hospice and palliative care services and is recruiting an RN Admission Nurse. The role meets with patients and families to explain hospice services and coordinate patient admissions. The nurse assesses patients at admission, arranges initial care needs, communicates with the Interdisciplinary Team, and completes required documentation. The position may also include nursing or “tuck-in” visits, training admission staff, and supervisory visits for LVN or CNA staff.

Requirements

• Registered Nurse licensed in the state of Texas.
• Valid driver’s license.
• At least 1 year of nursing experience.
• Case Management experience.
• Proficient computer skills and experience with Microsoft Office Suite.

Nice To Haves

• Strongly preferred: 2 or more years of hospice case management experience.

Responsibilities

• Conduct admission consultations and educate patients and families about hospice services.
• Complete a thorough patient assessment at admission; establish or arrange the initial frequency of nursing visits and determine DME and prescription needs.
• Assess the need for bereavement, social work, chaplain, and volunteer services.
• Attend and report to the Interdisciplinary Team regarding the patient.
• Complete paperwork and charting thoroughly and accurately according to policy.
• May conduct nursing or “tuck-in” visits.
• May train other admission staff on admission processes.
• May conduct supervisory visits for LVN or CNA staff.
