The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. This role involves completing clinical admissions for all Company services, explaining services to potential patients/families, identifying admission objections, educating on admission requirements, completing informed consents and initial assessments, providing information to Physicians, initiating skilled nursing interventions, obtaining Physician orders, and completing documentation per Company policy. The RN also acts as the Company representative at assigned facilities, facilitating referrals, communicating with referring hospitals and physicians, and providing care for GIP patients in a hospital setting. Responsibilities include representing the Company professionally, complying with regulations and policies, observing health and safety practices, maintaining confidentiality, using resources fiscally, promoting the Company, participating in performance improvement, advancing electronic media skills, supporting research and education, sharing expertise, and participating in Quality Assessment and Performance Improvement activities.
