The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. This role also involves completing clinical admissions for all Company services, explaining services to potential patients/families, addressing questions and concerns, educating on admission requirements, completing informed consents and initial assessments, providing information to Physicians and initiating the Plan of Care, initiating skilled nursing interventions, obtaining Physician orders, and completing documentation per Company policy. The RN acts as the Company representative at assigned facilities, facilitating referrals, working closely with referring hospitals, physicians, facilities, patients, families, and the general public. They communicate frequently with Admissions for timely follow-up, provide clinical communication using SBAR, discuss potential needs with after-hours staff, obtain consents for services, and arrange safe discharges while meeting productivity expectations. They also obtain medical record information and develop strong relationships with case managers and physicians. Additionally, they work with Professional Relations Representatives (PRRs) to educate on services and provide care for GIP patients in a hospital setting, including daily visits, professional management, documentation, and efficient discharge. The RN also provides and manages direct care to patients and families as part of the IDG, incorporating various components and appropriate nursing intervention and follow-up. They coordinate the Plan of Care, ensuring it reflects the patient’s evolving needs, and educate patients, families, caregivers, and other health professionals. The RN reports changes in patient condition, participates in evaluating hospice referrals/admissions, attends patient deaths, prepares bodies for transport, oversees medication disposal, and provides bereavement interventions. They present reports to the IDG, communicate accurately with physicians, staff, patients, families, and supervisors, and supervise patient care given by other nursing team members. During emergencies, the Triage RN may be required to report to work at a designated company location to ensure continuity of services, which may include reporting ahead of schedule and staying overnight.
