About The Position

We are a beautiful small 109-bed skilled nursing center with short-term rehab services. Full Time Monday through Friday 8:30am to 5:00pm We are seeking a highly skilled and clinically driven Registered Nurse (RN) to provide leadership and oversight for our Restorative and Safety Programs within a long-term care setting. This role is responsible for the clinical coordination, regulatory compliance, and operational management of restorative services, including the direct supervision and mentorship of Restorative Nursing Assistants (CNAs). You will ensure the delivery of evidence-based restorative interventions aimed at maintaining and improving residents’ functional abilities, while also leading facility-wide safety initiatives, fall prevention strategies, and quality improvement efforts. This position requires strong clinical judgment, data-driven decision-making, and the ability to oversee documentation accuracy, MDS alignment interdisciplinary care planning. The ideal candidate possesses a strong background in geriatric and rehabilitative nursing, demonstrates effective leadership in managing frontline staff, and is experienced in balancing clinical care delivery with administrative responsibilities, including audits, compliance monitoring, staff education, and program development. A collaborative approach and commitment to achieving positive resident outcomes and survey readiness are essential.

Requirements

  • Active RN license in SC
  • Critical thinking and problem-solving skills with attention to detail
  • Experience in long-term care, skilled nursing, or rehabilitation preferred
  • Passion for resident-centered care, safety, and quality outcomes
  • Working knowledge of restorative nursing and fall prevention programs
  • Familiarity with MDS, care planning, and regulatory requirements
  • Strong leadership, organization, and communication skills
  • Ability to train, mentor, and motivate frontline staff

Responsibilities

  • Lead and manage the Restorative Nursing Program to support resident independence and function
  • Perform safety rounds and identify environmental risks
  • Develop and monitor individualized restorative care plans (ambulation, ADLs, ROM, etc.)
  • Train and coach CNAs/restorative aides on techniques, documentation, and resident engagement
  • Partner with therapy and interdisciplinary teams to optimize outcomes
  • Drive fall prevention and resident safety initiatives through data tracking and interventions
  • Conduct incident investigations and implement corrective action plans
  • Ensure compliance with state, federal, and facility regulations
  • Support care planning, and survey readiness, MDS documentation
  • Participate in and contribute to QAPI/performance improvement activities
  • Serve as a clinical resource and champion a culture of safety and quality care
