About The Position
Join our team at Altoona Health and Rehab as a Restorative Nurse! Located at 6532 Walnut Grove Rd., Altoona, Altoona Health and Rehab is dedicated to providing high-quality care and support to our residents. We are currently seeking a skilled and compassionate Restorative Nurse to lead our restorative nursing program. As a Restorative Nurse, you will play a vital role in promoting resident independence, mobility, and overall well-being. You will work closely with our interdisciplinary team to develop and implement individualized restorative care plans for residents and oversee the delivery of restorative services.

Responsibilities
- Assessing resident needs and developing individualized restorative care plans to promote independence and functional ability.
- Implementing restorative nursing interventions, including range of motion exercises, ambulation programs, and activities of daily living training, to maintain and enhance resident mobility and function.
- Providing direct care and supervision to residents participating in restorative programs, ensuring safety and adherence to care plans.
- Monitoring resident progress and adjusting restorative interventions as needed to achieve desired outcomes.
- Documenting resident assessments, care plans, interventions, and outcomes accurately and timely in accordance with regulatory standards and facility policies.
- Providing education and training to nursing staff and certified nursing assistants on restorative care principles and techniques.
- Collaborating with therapists, physicians, and other healthcare professionals to coordinate care and optimize resident outcomes.
