About The Position

Under the direct supervision of a licensed nurse, implements and manages a restorative care program and provides restorative care to long term or skilled care residents. This role ensures that resident needs are met within the scope of practice and in accordance with facility policies and procedures.

Requirements

  • High school diploma or equivalent
  • Restorative nurse assistant certification (needs to be completed within 30 days of hire)
  • Valid Nevada State CNA certificate
  • Current CPR certification

Responsibilities

  • Identifies residents that would benefit from Restorative Care.
  • Establishes individualized restorative care plans that meet each resident's specific needs.
  • Establishes and adapts restorative care program schedule to meet changing needs of resident.
  • Assesses progress toward restorative care program goals.
  • Maintains ongoing restorative care care plans, reviews care plans at least quarterly and revises goals as necessary and appropriate.
  • Performs general strengthening exercises, PROM, AROM as directed by care plan established for specific resident.
  • Maintains daily documentation of resident involvement in restorative care program.
  • Collaborates with CNAs and nurses on a daily basis in regard to residents’ function and psychosocial status.
  • Observes and communicates changes in the residents’ condition to the charge nurse.
  • Works with RN supervisor to assess program effectiveness and suggest revisions as necessary.
  • Reviews and revises program policies and procedures annually.
  • Works closely with PT/OT to provide continuity of care recognizing a need to refer to a higher level of service and/or accepting transition from therapy services to the RC program.
  • Attends SNF quarterly QAPI meetings to report status of restorative care program.
  • Demonstrates sensitivity to patient comfort and privacy and expresses interest in their progress.
  • Maintains a safe, comfortable, and therapeutic environment for residents, families and visitors in accordance with hospital standards.
  • Performs personal care assistance to resident which includes dressing, eating, toileting, ambulation, transferring, lifting, bed positioning, or any other function of their care that assists the floor on CNAs as necessary.
  • Contributes to overall resident care planning process.
  • As a member of the healthcare team, maintains harmonious relationships with the unit and co-workers providing support and assistance to co-workers to ensure quality care.
  • Supports the department by attending and constructively contributing to staff or other meetings as able/necessary.
  • Communicates clearly and tactfully with resident and their families.
  • Maintains confidentiality of all residents/staff information.
  • Performs other related duties as requested or assigned.
