About The Position

The Respite Navigator provides case management support to adults in the community with complex mental health needs. Many of these individuals need assistance connecting to community resources such as housing, finance, employment, transportation, community providers, etc. The Navigator will act as the primary point of contact for community providers and will conduct outreach, engagement, assessment, and person-centered treatment planning for individuals who are enrolled in the Mobile Respite program. The Respite program is a short-term group living environment that provides treatment of intermittent crisis and stabilization of adults with are diagnosed with a mental health/substance use disorder. The goal of the program is to work collaboratively with individuals in developing skills that allow them to live independently and successfully in the community.

Requirements

  • High School Diploma/GED or equivalent, required.
  • Applicable experience in community-based or inpatient behavioral health, required.
  • Experience working with adults diagnosed with mental illness/substance use disorders.
  • Experience with accessing local resources, navigating health, behavioral health, and/or substance abuse treatment systems.
  • Experience working with community partners such as: Hospitals, Crisis Support Programs, Law Enforcement, Medical/Mental Health Providers, and/or DMH is required.
  • Valid driver's license, acceptable driving history and reliable transportation, required.
  • Ability to work independently is required.

Responsibilities

  • Provide case management support to adults in the community with complex mental health needs.
  • Assist individuals in connecting to community resources such as housing, finance, employment, transportation, and community providers.
  • Act as the primary point of contact for community providers.
  • Conduct outreach, engagement, assessment, and person-centered treatment planning for individuals enrolled in the Mobile Respite program.
  • Collaborate with DMH funders regarding referrals.
  • Establish and maintain positive professional relationships with applicable funding, referral, regulatory, and other community agencies.
  • Review and complete all required documentation.
  • Acquire and maintain all required certifications.
