About The Position

This role assesses clients' housing, education, employment, and resource needs, develops individualized intervention plans, and connects clients with appropriate community services. The position collaborates with treatment teams, maintains community partnerships and resource networks, coordinates transportation and support services, participates in outreach efforts, and ensures accurate and timely documentation in the Electronic Health Record (EHR) system. The population served includes adults, 18 plus, with outpatient community-based services and support delivered throughout Fresno County. The role may require transporting clients/client family members utilizing a personal vehicle. Clinical services include individual counseling, medication management, housing, education, and vocational support services.

Requirements

  • High School completion required.
  • Valid California Driver’s License.
  • Eligibility to meet agency driving criteria.
  • If Associate's degree: two (2) years of experience performing non-professional, consumer directed services in behavioral health, healthcare, or social services.
  • If High School completion: an additional two (2) years of experience in a social service field.

  • Associate’s degree in a Social Services field preferred.
