Employee Recognition & Rewards : A culture that celebrates and rewards your hard work and dedication How you will make a difference : Responsible for helping residents meet service and resource needs including housing, vocational/educational services, transportation, scheduling appointments, and other social support services. Responsible for identifying housing resources and developing relationships with community agencies. Assists clients in completing facility consent forms upon admission Actively participates in treatment team meetings and is responsible for working with other disciplines to develop a coordinated treatment plan for assigned clients. Provides consultation as needed with families and the Case Manager on issues regarding vocation, education, and housing. Documents social services that are provided and records them in the patient chart. Assists with transportation to appointments to provide direct linkage to resources as needed Division/Program Overview The TAY Crisis Residential Treatment (CRT) facility is a short-term voluntary residential facility. Designed to help Transitional Age Youth (TAY), ages 18-25, who experience a psychiatric crisis to safely stabilize, recover, and transition back to community. Services include: · 24/7 program · Therapeutic and mental health services · Rehabilitation and recovery services (including substance rehabilitation) · Pre-vocational or vocational counseling · Medication evaluation and support services · Daily exercise and health/wellness education · Crisis intervention.
