TWIN OAKS JUVENILE DEVELOPMENT INCTallahassee, FL

About The Position

The Resource Navigator (RN) serves as the primary program contact for youth, parents, Facilities, and Juvenile Probation Officers (JPOs) from case assignment through discharge. The RN collaborates closely with the JPO, Residential Case Manager (RCM), youth, and parent/guardian to complete required transition contacts, including participation in the Transition Conference, Community Reentry Team (CRT), and Exit Conference. This role involves completing all required transition service contacts, both by telephone and face-to-face, during transition and post-release. The RN actively participates in monthly internal Navigator Team Meetings, informing the transition team of youth needs. They are the primary contact for communication with providers for Service Tier 1 referrals, while the Resource Navigator Coordinator (RNC) handles Service Tier 3 and 4 referrals. The RN collaborates with the JPO, Facility, youth, parent, and referral resources to develop the "Transition Plan for Success" before release. They identify service categories of need for the youth to recommend Service Tiers and update referral resources. The RN confirms required services, referrals, and intake appointments with the parent and JPO, and develops recommended goals for the "YES Plan" with input from various stakeholders. Weekly contact with the youth and family is maintained to review YES Plan goals and progress, with oversight from the Circuit's Resource Navigator Supervisor. Upon completion or substantial progress on YES Plan goals, the RN prepares the Discharge Summary Report for discharge approval.
