About The Position

The Grand Valley Connects Resource Navigator works in the community providing high-quality customer service, support, and education to community members. This program offers a centralized system to maximize efficiency by increasing awareness and normalizing access to needed services. The core goal of this program is to assist Mesa County residents as they navigate potential services to fit their current needs and enrollment processes. Resource Navigators will use critical thinking and creative problem-solving skills to assist community members with identifying potential community resources to address needs, assisting in the enrollment processes, advocating on their behalf as needed, and helping navigate barriers to services that members may experience.

Requirements

  • High School Diploma or the equivalent.
  • A valid Colorado driver’s license with a responsible driving record.
  • Advanced knowledge and ability to use Microsoft Office applications including, Word, Excel, and PowerPoint.
  • Advanced knowledge and ability to use Google Applications including Gmail and Drive.

Nice To Haves

  • Associates Degree in human service or public health related fields preferred.
  • A minimum of 1 year of relevant experience preferred.
  • English/Spanish bilingual highly preferred.
  • Any combination of education, training and experience which provides the knowledge, skills and abilities required for the job.

Responsibilities

  • Respond in a timely manner to community member inquiries received by phone, in person, or through email/text.
  • Using a whole person approach, complete assessments of community member needs and identify potential and appropriate programs and services to address those needs.
  • Identify potential and appropriate programs and services to address community member needs and reduce barriers to economic stability and health stability.
  • Assist with the completion of application forms or other paperwork necessary to connect community members with available programs and services, including government benefits (SNAP, WIC, CCAP, Medicaid, etc), available tax credits, medical/dental providers, etc.
  • Assist community members with acquiring basic documentation like birth certificates, Social Security cards, Driver’s Licenses, and IDs.
  • Maintain a caseload of up to 55 active community members, including the initiation of follow-up contacts at specified intervals and additional ongoing support at the request of community members.
  • Actively conduct ongoing research to identify available programs and services, and seek to understand eligibility criteria and application processes for those resources.
  • Record detailed program and service information and update information as appropriate in applicable systems.
  • Provide opportunities for community education on available community programs and services to increase awareness and normalize enrollment.
  • Maintain confidentiality of all member information.
  • Participate in the establishment of program goals, objectives, and benchmarks with Program Supervisor/Division Director and evaluate progress toward attaining goals.
  • Represent Mesa County Public Health positively by offering exceptional customer service in responding to community needs, enhancing public relations and coordinating community activities
  • Maintain understanding of all program, department, and county policies and procedures.
  • Perform rotating office coverage with other Grand Valley Connects staff.
  • Work public events, evenings, and weekends, as needed.
  • Perform all other duties as assigned.
  • Participates in emergency planning and response activities.
  • Participates in agency-wide activities that may include strategic planning, quality improvement, and/or staff development, as appropriate.
  • Completes necessary training through FEMA, including Incident Command Structure (ICS) courses.
  • When a local declaration of emergency or disaster is declared by County leadership, employees may be required to work as part of a National Incident Management System (NIMS).
