About The Position

Under the supervision of the program supervisor, this role identifies, recruits, and interviews families to assess the health and social service needs of families and children to determine their level of risk. The specialist conducts outreach and in-home recruitment activities for expecting and new parents. They also conduct screenings, in-home assessments, and refer families to community resources, including the Healthy Families New York home visiting program.

Requirements

  • High school diploma or general education degree (GED).
  • One year of experience in working with or providing services to high-risk or low-income children and families in a home community setting.
  • Ability to perform each essential duty satisfactorily.
  • Ability to make sound decisions pertaining to safety, personnel, and programmatic boundaries.
  • Ability to be culturally sensitive with respect to other individuals.

Responsibilities

  • Identify, recruit, and interview families to determine level of risk, obtain informed consent, and refer families to appropriate resources including home visiting services.
  • In collaboration with the family, explore health, social service, and educational needs, make appropriate referrals to other agencies, and follow up on all referrals, acting as an advocate when needed.
  • Implement the outreach and recruitment plan as directed, making community contacts, and creating and disseminating marketing materials and other program information.
  • Pick up surveys and screens from various referral sources within the county.
  • Maintain accurate records of outreach, family contact, and assessments utilizing a management information system.
  • Maintain up-to-date knowledge of community resources.
  • Maintain confidentiality of all acquired information.
  • Observe and report accurately on the functioning of individuals and families.
  • Demonstrate knowledge of infant and child development.
  • Prepare written reports in a timely manner.
  • Make sound decisions pertaining to safety, personnel, and programmatic boundaries.
  • Be culturally sensitive with respect to other individuals.
  • Attend meetings, case reviews, staff meetings, parent meetings, in-service, and other meetings/trainings as directed.
  • Participate in regularly scheduled and protected supervision.
