About The Position

This opening is with our adult residential Bridge Program. The Bridge is a 12-bed residential treatment program for homeless adults living with a major mental illness, many of whom also experience co-occurring conditions. The program works to address the complex needs of the clients, utilizing an integrated, individualized, and client-centered approach, operated 24 hours/day by staff trained in trauma-informed care, crisis intervention, and mental health and substance use treatment. Often our clients have co-occurring conditions, such as chronic health conditions and/or substance use disorders. The Transition Coordinator is a technical position that provides direct services in support of adults transitioning in and out of residential programs. This position contributes to the accomplishment of the Agency mission through the provision of direct care, contributing to the ongoing co-occurring treatment plans of the program. Work is performed, reviewed, and evaluated under the supervision of the Program Director.

Requirements

  • High School Diploma required.
  • Must become certified as Mental Health Rehabilitation Technician 1 (MHRT1) within 1 year of employment; then maintain required trainings to uphold continuous MHRT1 status.
  • Contracts that have Rider D: Must be able to successfully pass a criminal background, child protective service check & sex offender check.
  • Must not be on the state or federal suspension and disbarment list.
  • Must pass CNA registry check
  • Must be able to pass pre-employment physical and TB screen.
  • Must be able to exert moderate physical effort with occasional standing, walking, reaching, stooping, bending, kneeling, crouching, typing and frequently lift and carry small children or material weighing up to 25 pounds.
  • Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area. This may include transportation of clients or driving to agency location to complete work.
  • A good driving record is required.
  • This role is required by the State of Maine to have proof of immunization or documented immunity against rubeola (measles), mumps, rubella (German measles), varicella (chicken pox), hepatitis B (optional), and influenza.
  • Medical exemption may be accommodated with a written statement from a licensed medical professional.
  • The Agency may assist with obtaining vaccines or proof of immunity if needed.

Nice To Haves

  • Associates degree preferred.
  • Two years’ experience in a mental health setting preferred.
