About The Position

4C Health is a growing non-profit Community Mental Health Center currently serving the communities of North Central Indiana since 1975. 4C Health is deeply committed to increasing access to a continuum of care while integrating physical and behavioral health well-being for our staff and consumers. This position develops, maintains and oversees a system of residential care for seriously mentally ill adults that is responsive to both the needs of clients and community expectations and consistent with guidelines for best practice interventions. This position works directly with the Director of Adult Intensive Services to maintain fiscal and operational excellence as well as ensure all accreditation requirements are fulfilled. Additionally, this position is responsible for promoting mental health recovery through a person-centered approach focusing on independence and integration. This is accomplished through delivery of appropriate mental health assessment, treatment planning and intervention for clients impacted by emotional/behavioral impairment and serious, persistent mental illness who are targeted for mental health services by the Division of Mental Health. This position operates under minimal supervision and must exercise tact, judgment and confidentiality in contact with the public, clients and families, as well as fellow associates of the Center. Work is very broad in scope and requires a high level of professionalism.

Requirements

  • A Bachelor or Associate degree preferred.
  • High School diploma with 6 years of related experience.
  • A degree is social services, psychology or related mental health field preferred.
  • At least six (6) college credit hours in abnormal psychology or other related psychology classes preferred.
  • If degree is in a field other than that previously stated, must have five (5) years of work experience in mental health or social services.
  • Previous experience with case management, skill building, and group therapy or in a group home setting preferred.
  • Experience working with children and adults who demonstrate emotional and psychiatric disturbance and behavioral needs preferred.
  • Leadership experience preferred.
  • Must have a valid Indiana Driver’s License and maintain vehicle insurance requirements per Center policy.

Nice To Haves

  • Previous experience with case management, skill building, and group therapy or in a group home setting preferred.
  • Experience working with children and adults who demonstrate emotional and psychiatric disturbance and behavioral needs preferred.
  • Leadership experience preferred.

Responsibilities

  • Complete client assessments that include intake, evaluating a client’s ability to meet the admission criteria for that particular department, identify presenting problem(s), history, medical information, family involvement, psychosocial/psychosexual information, and other pertinent information to form an appropriate clinical impression including substance use/abuse and abuse/neglect.
  • Complete a treatment plan, which accurately reflects the client’s needs and ability to meet discharge criteria with goals and objectives that are time limited and behaviorally stated.
  • Document timely, accurately and in accordance with Center policies utilizing current systems and practices.
  • Document accurate information, which reflects client progress based on the appropriate therapeutic modality.
  • Therapeutic services will be time limited and specific to the presenting problem(s) of each assigned client.
  • Provide progress reports to referral sources as needed for each individual client.
  • Completes all necessary and required monthly progress reports and ensures appropriate care coordination.
  • Periodically communicate with the referring agency, at least monthly, to provide progress updates and obtain relevant collateral information from the referral source and other external treatment team members.
  • Participate in treatment team meetings with others involved in the client treatment plan (internally and externally).
  • Provide skills training and development as a primary intervention to clients making sure that skills training is based on each individual client and their vision for recovery.
  • Provide direct service case management and skill building intervention for referred clients and their families addressing goals and objectives as outlined on the treatment plan.
  • Provide clinical client contact hours based on billable counts of service, FTE and area of services as determined by Director of AIS.
  • Meet RVU goals as set by the department/supervisor.
  • Obtain food handlers’ license.
  • Complete On Call Schedule Group Home
  • Assist in CCS initial and ongoing training and support.
  • Responsible for Medication Count Tracking
  • F1 Forms
  • Responsible to track Lease Agreements
  • Cosign as needed.
  • Handwashing observation and Lice checks.
  • Completion of cleaning forms and ensuring cleanliness of homes
  • Submit GH related expenses and receipts (groceries, etc.)
  • Completing incident and DMHA reports as needed.
  • Assist with supervisions as needed.
  • Work evenings and weekends as assigned or necessary.
  • Participate in an on-call rotation.
  • As CPI trainer, conduct CPI trainings as needed by the agency and providing coverage for other CPI trainers in the organization and support crisis drills as needed for 24 hour programs
  • Meet with assigned clients and/or families as frequently as outlined in the treatment plan.
  • Transport clients to appointments or other destinations (relevant to the treatment plan).
  • Assist clients in the development of natural support systems to foster independence as much as possible.
  • Participate in continuing education pertaining to case management, mental health, addictions or other areas applicable to services provided and populations served.
  • Complete required supervision (individual and group) per Center policy.
  • Utilize the Wellness Recovery Action Plan as applicable with clients.
  • Provides supervision to GROUP HOME staff, manages the interdisciplinary treatment team process, and ensures quality of care for treatment provided.
  • Creates and supports the recovery philosophy.
  • Demonstrate understanding of fiscal and operational needs of Group Homes.
  • Responsible to assist Director of AIS in the onboarding process and post initial orientation training to ensure strong mentoring program and development of Group Home staff.
  • Educate, train, and support clinical staff in areas of practice management and provision of excellence in clinical care in group homes.
  • Oversee and ensure appropriate staffing levels at all times in the group homes.
  • Abide by all Center policies and procedures.
  • Maintain positive working relations and rapport with the communities served.
