About The Position

The Hampton - Newport News Community Services Board is hiring a Residential Nurse (LPN) to support individuals served in our Intermediate Care Facilities and Waiver Homes by coordinating and delivering nursing services that promote health, stability, and safe medication practices. This role ensures timely access to medical care, maintains accurate clinical records, and provides guidance to program staff and families.

Requirements

Licensed Practical Nurse (LPN) license in Virginia
Graduate of an accredited LPN program
One (1) year of clinical experience including medication administration (community program, hospital, or nursing home)

Experience in an Intermediate Care Facility, Waiver Home, or working with individuals living with developmental disabilities

Responsibilities

Nursing assessments
Development and implementation of nursing care plans
Coordination of referrals, transportation, and attendance at medical appointments
Medication management oversight (storage, administration, and destruction)
Crisis intervention as needed
Consultation/education to staff on medical concerns
Participation in interdisciplinary service planning
Adherence to safety, environmental, and infection control standards
Reviewing overnight updates and current health priorities
Checking in with clients to note changes in medical or behavioral status
Connecting with physicians, pharmacies, and therapy providers to confirm orders and coordinate upcoming visits
Documenting assessments and plan-of-care updates
Responding to time-sensitive concerns
Providing supportive counseling to clients and families
Answering clinical questions for staff
Offering brief coaching to staff
Helping the team stay aligned with safe practices
Preparing for on-call coverage
Ensuring records are complete and ready for the next shift
