Bancroft is currently seeking an experienced Residential Nurse Case Manager to join our Cherry Hill, NJ team. This role promotes the health and wellness of the individuals supported, including services, advocacy, and education to contribute to the continued provision of quality healthcare to persons with developmental disabilities, brain injuries, and neurological impairments within the specific program. The position operates within personal license constraints in strict accordance with regulations and guidelines defined by various state and federal agencies, as well as established Bancroft policies and procedures. The Nurse Case Manager provides various nursing care management oversight and identifies, assesses, and improves and maintains the quality of life and healthcare for persons served. This role serves as a key liaison, advocate, and resource of information regarding both routine and special healthcare needs to support a comprehensive care plan for each person. The position facilitates a team approach to the continued provision of quality care through interaction, communication, and collaboration with a wide range of stakeholders including family members, clinical staff, physicians, healthcare providers, pharmacists, management, program staff, school staff, external caseworkers, regulatory agencies, insurance companies, and hospitals.
