About The Position

Under the direction of the Community Residential Rehabilitation (CRR) Residential Supervisor, the Mental Health Worker is a direct care position that will provide for the safety and well-being of each resident within the home and facilitate community involvement/interaction in cooperation with the residents, staff and administration. The CRR Mental Health Worker will facilitate implementation of general and specific household and individual routine/programming while serving as a role model for the resident population. In partnership with the CRR Residential Coordinator, the CRR Mental Health Worker provides a continuum of services, including system advocacy, informational/referral, supportive counseling, family and resident education, and crisis intervention / stabilization.

Requirements

  • Must be able to work 1pm-9pm shifts, split between 2 sites.

Responsibilities

  • Uphold the philosophy, policies and mission statement of CGRC.
  • Provide supervision, guidance and training in accordance with individual needs and goals.
  • Arrange activities, preparing and organizing individual and/or group activities and creative skill building.
  • Plan food / meal coordination, work closely with all residents in a unified manner to keep their home neat, clean and odor free and insures that all laundry is completed properly. Will complete task if resident cannot.
  • Provide money management supervision, works directly with the resident in an effort to build skills for achieving independent financial abilities.
  • Ensure home maintenance and safety awareness.
  • Accurately assess resident needs and documents in bi-monthly review.
  • Monitor service provision through intensive case management to ensure receipt of quality and appropriate services/interventions.
  • Develop therapeutic alliances with adults, works with residents to resolve issues of therapy relationships.
  • Incorporate understanding of medication management as it supports the overall clinical plan.
  • Supervise the self-administration of client medication. Ascertain through questions that clients’ understand his/her prescribed medication – dose, side effects, etc., as guided by a registered nurse.
  • Manage an effective team partnership, including co-therapy, team communication and role flexibility.
  • Provide information / referral services to clients and families to ensure receipt of quality and appropriate services/resources.
  • Adhere to all agency corporate compliance mandates including accuracy, timeliness and attention to detail.
  • Complete all documentation requirements within determined timeframes; completes all forms, consistent with CGRC corporate compliance procedures.
  • Complete all training and actively participates in required supervision in accordance with program and agency guidelines.
  • Performs other duties and special projects as assigned.
