About The Position

The Medical Coordinator, under the supervision of the Residence Manager, will work with the RN of Block Institute programs to ensure quality medical care for individuals diagnosed with Intellectual and Developmental Disabilities. The Medical Coordinator will track and monitor processes related to the health care of individuals provided in a residential setting as well as provide needed assistance to individuals with Intellectual and Developmental Disabilities in all areas of daily living.

Requirements

  • At least 5 years’ experience within an OPWDD residential setting
  • Valid Driver’s License with no more than 4pts or one (1) recorded at fault accident in a 12-month period.
  • High School Diploma or GED Equivalent
  • AMAP Certification in good standing for at least 3 years
  • Strong Oral Computer and Written Skills
  • SCIP and CPR Certified
  • Commitment to ongoing training

Responsibilities

  • Conduct weekly medication inventory, ensuring orders and deliveries are completed.
  • Oversee AMAP medication inventory checks and ensure controlled medications are counted and documented daily.
  • Review MARs daily for missing signatures, medical charting (BMs, BP, fingersticks), and prescription accuracy.
  • Schedule and track medical appointments, lab tests, and diagnostic procedures in coordination with the RN.
  • Ensure all prescriptions are documented correctly in MARs, and discontinued medications are properly disposed of.
  • Prepare monthly MAR sheets with accurate medication details and maintain CVS and MIPS forms for medical visits.
  • Monitor AMAP certifications, medication errors, and training compliance, reporting updates to the RN.
  • Attend and advocate for individuals at medical appointments, ensuring necessary adaptive equipment and documentation are in place.
  • Communicate medication changes and appointment outcomes promptly with the nurse and management.
  • Maintain medication storage, inventory tracking, and AMAP communication logs for compliance.
  • Train new AMAP staff and perform DSP duties as needed to ensure the safety and well-being of individuals.
